Slezak, John W., “NY John”, 61, Johnstown, died February 10, 2014 at Memorial Medical Center. Born July 21, 1952 in Jackson Heights (Queens), NY, son of the late John and Helen (Sabo) Slezak. John shared in a loving union of 25 years with Michelle Smith and her family, Aunt Joann; Jan Borsek; Frank Boyd; Neil Penrod and their families; sister, Patricia Buttenhoff and her son, Bill Pesek and family; sister, Sophie Carroll and son, John J. Slezak. John was a driver for Bio Test Medical, he loved music and attended Woodstock in 1969, everything New York including the Yankees; Jets and Katzs and loved Pennsylvania for the fishing and outdoors. He will be loved and missed by many friends. At the request of the deceased there will be no viewing. A private service will be held at the convenience of the family.
