MILLARD – John Andrew, 44, Salix, died October 18, 2011. Born March 31, 1967 in Johnstown; son of Winfield H. and Barbara A. (Corby) Millard. Preceded in death by maternal and paternal grandparents. Survived by parents; son, John, Windber; brothers, Brian, Portage; Brett, Statford, VA; and Brandon, Elton.
